Some Helpful Forms…

Below are some helpful forms that can be useful to complete and bring to your consultation.

K10

The K10 is a 10-point questionnaire which screens for symptoms of anxiety and depression. If you are attending for a new mental health care plan, to review an existing mental health care plan or with concerns about your mental health, it would be very helpful to complete and bring this, or the DASS 21 to your appointment.

Blood Pressure Form

You can access a form here to records one week of your home blood pressure readings for your GP.

DASS 21

The DASS 21 is an in-depth scoring system which screens for anxiety and depression. It could be helpful to complete this, or a K10, prior to attending your appointment to discuss your mental health or mental health care plan.

EPDS

The Edinburgh Postnatal Depression Scale is a questionnaire designed to screen for postnatal depression. It is really useful to complete it and bring to your six-week postanal check up with your GP. It can be helpful to fill in and discuss with your GP if you have had a baby in the last few years and have concerns about your mental health.

EDE-Q

The Eating Disorder Examination Questionnaire screens for symptoms of an eating disorder. Eating disorders are common and include anorexia nervosa, bulimia nervosa and binge eating disorder. If you are concerned that you, or someone you know may have an eating disorder it might be worth you/them filling out an EDE-Q and taking it to discuss with your/their GP.

ADHD Self Assessment

The Attention-Deficit Hyperactivity Disorder self-report scale is a useful tool to screen for symptoms of ADHD. If you are concerned you may have ADHD then it might helpful to complete this form and bring to an appointment with your GP to discuss.

Feeling sleepy?

It might be worth completing an ESS or STOPBANG to discuss with your GP

ESS

The Epworth Sleepiness Scale is a scoring system used to assess fatigue levels. It is commonly used to screen for the possibility of having a condition called obstructive sleep apnoea. If you snore, have pauses in your breathing, wake regularly during sleep, feels tired all the time or have unrefreshing sleep you might want to complete and ESS and take it to your GP appointment.

STOPBANG

The STOP-BANG screening tool it used to look for symptoms of, and risk factors of obstructive sleep apnoea. If you aren’t sure what your BMI or neck circumference is don’t worry, your GP can measure these at your appointment.

 

Iron Infusion

Sometimes, your GP may recommend an iron infusion. Below you can access an information leaflet.
